For appointments on or after February 28, 2021, you must meet any of the Eligibility Criteria in categories (1) – (5) below.
healthcare workers who live or work in Santa Clara County; or
residents of Long-Term Care Facilities (LTCF) located in Santa Clara County; or
individuals aged 65 or above who live in Santa Clara County; or
individuals aged 65 or above who receive their primary care from Santa Clara Valley Medical Center Hospitals and Clinics; or
Residents of Santa Clara County who are at risk of exposure to COVID-19 through their work in:
Education and Childcare
Emergency Services
Food and Agriculture including those working in a restaurant
